In order to be approved as a Volunteer Coach or Chaperone, there are THREE REQUIRED CLEARANCES you must submit:

1. PA State Police Background Check (PATCH)

2. Pennsylvania Child Abuse History Certification

3. FBI Criminal History Record -OR- Disclosure Statement for Volunteers *SEE BELOW

*If a Volunteer has been a resident of the State of Pennsylvania for the past ten (10) consecutive years, he/she can complete the Disclosure Statement Application for Volunteers located below in lieu of completing the paid FBI Certification. Those who have lived in PA less than 10 years need to complete the FBI Certification.
